Resume for Colleges	This can be used to prepare a resume or summary of activities for colleges or employers.
Personal Resume for Teachers and Recommendations	This is a different version of the form shown above.
Microsoft Word Resume Form	You can create a personal resume in MS Word by following this path: File a Newa General Templatesa Other Documentsa Resume Wizard. You will be able to customize your entries for a resume.
Electronic Transcript Request	If you submit a college application electronically, you need to order a transcript from your counselor. Print this form and give it to your counselor.
Transcript Release Form for Class of 2008	We must have a signed copy of this form on file to mail or release a high school transcript.
Transcript Release Form (for graduates or non-seniors)	Do not use this form if you are currently enrolled as a senior.
Prairie State ACT Release Form	This form must be on file with us if you want us to have your Prairie State ACT scores included on your transcript for Fremd High School. Print this form and have your son or daughter return it to us or fax it to the attention of the Registrar. (847-755-2904)
